Coudersport, PA– For over 25 years Cole Memorial’s Home
Health, Hospice and Homemakers teams have provided care to Potter County and
surrounding areas. During the past few
years their office was located beside the PA Careerlink building at 279 Route 6
West in Coudersport but recently moved to the Zito Media facility at 102-108
South Main Street.
The agencies’ staff of approximately 70 highly-skilled
healthcare professionals and volunteers will remain the same and their
telephone/fax numbers will not change.
Most major health insurance plans will continue to be accepted including
Medicare and Medicaid.
Cole Memorial’s, Joint Commission Accredited Home Health
& Hospice agency, provides a full-range of 24-hour healthcare services for
patients of all ages in their homes, including:

services managed by a registered nurse
· Coordinated
care with your medical provider
· Home Health
Aides for daily living activities
· Hospice
· Infusions
· Medical
social work

Rehabilitation services
· Palliative
care and caregiver support via the Hospice program
· Medical
equipment, oxygen and supplies delivered to homes from Cole Care.
The Cole Memorial Homemakers program is licensed by the
Pennsylvania Department of Health to offer individuals help with bathing and
other nonmedical personal care needs. They also offer home support, which
includes light housekeeping, meal preparation and errands.
