Ardent Solutions, in conjunction with the Literacy West is pleased to announce the upcoming Diabetes Prevention Program for anyone 18 years of age or older, at-risk or diagnosed with pre-diabetes.
The program will begin March 7, 2018; 11:00 a.m. – 12:00 p.m. and continue for sixteen weeks, on Wednesday’s, through June 27, 2017, at the Literacy West in Belmont. To register for this program, please call Patty Amidon, with Ardent Solutions, Inc. at 585-593-5223, ext. 1010, prior to March 5, 2018. This program is FREE and each participant will receive all the materials for the 16 week class and post-core sessions. Upon completion of the lifestyle change program, participants will receive a $50 gift card.
Based on effective efforts researched by the National Institutes of Health and the Centers for Disease Control and Prevention, the Diabetes Prevention Program will help participants learn about and adopt the healthy eating and physical activity habits that have been proven to reduce the risk of developing Type 2 Diabetes. Individuals will receive support and encouragement from both a trained Lifestyle Coach and fellow classmates as they develop a plan for improving and maintaining their overall health and well-being.
The 12-month, group-based program consists of 16 core sessions conducted weekly in community-based settings, worksites or healthcare facilities. To stay motivated and maintain progress towards program goals, participants continue with monthly maintenance sessions.
