Ardent Solutions invites volunteers to participate in the free upcoming evidence-based Peer Leader training programs developed by Stanford Patient Education Research Center, Chronic Disease Self-Management Program (CDSMP) and Chronic Pain Self-Management Program (CPSMP). We are accepting a wide range of applicants, from individuals with healthcare education and experience to those with little or no experience.
The CDSMP training is scheduled for Tuesday, August 14 – Friday, August 17, 2018; 8:30 a.m. – 4:30 p.m. The CPSMP training is scheduled for Tuesday, August 21 and Wednesday, August 22, 2018; 8:30 a.m.-4:30 p.m. Both trainings are held at Jones Memorial Hospital, located at 191 North Main Street, Wellsville, NY. Please note, individuals wishing to take the Chronic Pain Self-Management Training (CPSMP) are required to complete the 4-day CDSMP training prior. Materials, continental breakfast, lunch and technical assistance for all future classes will be included with the trainings.
As a trained Peer Leader, you commit to delivering two (2) workshops each year. These evidence-based workshops originally developed by Stanford University run for 2.5 hours, once a week, for six consecutive weeks and is led by two Peer Leaders. The health education workshop helps participants develop new tools and skills to manage the symptoms and feelings that can result from living with a chronic health condition or chronic pain. Participants become more confident about managing their own chronic condition or that of a loved-one. Workshop topics include: action planning, problem solving, physical activity, nutrition, relaxation techniques, positive thinking and working with healthcare providers
Pre-Registration is required by Thursday, August 9, 2018 by contacting Patty Amidon at 585-593-5223 ext. 1010 or via email at amidonp@ardentnetwork.org.
