Community Needs Assessment Survey

As part of ACCORD’s 2025 Community Needs Assessment (CNA), we’ve launched a series of surveys to better understand what matters most to our communities. Your input—and your help in spreading the word—plays a vital role in shaping the programs and services that respond to real needs across Allegany County.


There are three separate surveys:

  • One for community members
  • One for staff, board members, and volunteers
  • One for community partners and collaborators

The Community Member Survey is the one we are actively promoting throughout Allegany County. We’ve included a flyer you can post, share, or print (full-page and half-sheet formats included) to help get the word out.

  • Print and distribute the flyer using either version attached

→ For external organizations, agencies, and community collaborators
