The Rotary Club of Wellsville will be sponsoring the Annual Blood Analysis Wellness Program, as it has for decades, on October 10, 2020 from 6-10 am at Grace United Church, 289 North Main Street, Wellsville. We are especially pleased to be able to offer this service during the turbulent year of 2020, as the annual tracking of data from these tests provides useful comparative data for your physician. This blood analysis covers coronary diseases, kidney diseases, anemia, liver diseases, diabetes and assorted other tests. The cost is $41, and this screening is not covered by Medicare and most other insurance companies. PHI forms required for patient copies, photo ID required. Other tests, including prostate Specific Antigen, TSH, Vitamin D, Hemoglobin A1C, and Insure Colon Rectal kit are available for additional fees. Personal physician authorization is required. A separate physician's order is required for any of the optional tests. Masks will be required, and the traditional doughnuts cannot be offered, as the precautions of Corona virus dictate. Checks or money order only will be accepted the day of the program and should be made payable to AMBA Wellness Programs.
