News Release: Allegany County Department of Health Launches Community Health Survey

UR Jones Memorial Hospital, Cuba Memorial Hospital, ACDOH and the Community Wellness Committee are looking for your input!

BELMONT, NY – Your Opinion Matters about the health of the community and the services that are offered.  Completing this short survey is a critical step for improving the health of residents across Allegany County. By offering your opinion, it helps the Health Department understand your needs, which strengthens the health of our community. It is important to know that this short survey guides new programming by multiple community health partners.  

 

Your Time…Your Way. This survey is available in a variety of formats:

·         Paper: Available at the ground floor of the County Office Building and entrance to the County Courthouse located at 7 Court Street, Belmont; UR Jones Memorial Hospital, at the UCC Registration area and Switchboard inside Cuba Memorial Hospital, the David A. Howe Public Library in Wellsville, and the main lobby at Office for the Aging.

Why is this important? Completing the survey will make us aware of the hurdles you cross in your everyday life. At times, there are barriers for people to maintain a healthy lifestyle, and hearing about these challenges through this survey allows us to develop strategies that lead to actions as we work diligently to move the dial toward healthier communities.
